Carl's Results Report

I should accurately document Carl’s results. According to the Code of Ethics of the National Association of Social Workers, under Social Workers’ Ethical Responsibilities in Practice Settings 3.04 Clients Records states, “Social Workers should take reasonable steps to ensure that documentation in records is accurate and reflects the services provided.” Additionally, documenting Carl’s results is directly relevant to the delivery of services. Carl is a veteran and he is receiving services at a treatment program I run, so I have no obligation to the Uniform Code of Military Justice. It’s my duty to document, but I cannot release his records without his valid consent.
