Case Study Aetna Contract
I have completed a preliminary review of the Aetna contract as requested. I have four areas of concern I would like to briefly discuss. I believe all warrant further scrutiny.

Under the participating provider staff privilege clause, Orlberg would be required to offer privileges to qualified providers from outside the current medical staff. These providers may be from near-by hospitals and direct competitors to Orlberg’s current medical staff. In addition, this potentially could lead to a conflict with Orlberg’s current exclusive provider arrangements.

I am concerned that in section 2.10.4 that payment for an admission ends when Aetna determines the day of discharge. This may conflict with physician judgment and lead to uncompensated

As the CFO of Community Memorial Hospital, I have a major problem on my hands that could lead the hospital in a financial bid. Bill Jacobs, the Human Resource Director of Commercial Intertech (CI) which is the largest employer in the community, has signed a contract MegaPlan Health. MegaPlan is a known insurance company that gives hospitals a very hard time especially when it comes to discounts and fighting claims the hospitals make. Since all of our employees and their families will be using MegaPlan Insurance and Community Memorial is not on their Preferred Provider Network (PPN), I need to figure out if we should work with or without MegaPlan. If so, I will have to decide to sign this contract that they sent over that includes a 35% discount from charges and service pre-authorization requirements or write a contract of my mine with all of our terms and conditions and see if they agree to it. Regardless of the decision I make, there will be major risk involved working with MegaPlan Insurance.…

On August 7, 2014, the President signed into law the Veterans Access, Choice, and Accountability Act of 2014 (“the Act,” Public Law 113-146, 128 Stat. 1754). Further technical revisions to the Act were made on September 26, 2014, when the President signed into law the Department of Veterans Affairs Expiring Authorities Act of 2014 (Pub. L. 113-175, 128 Stat. 1901, 1906). Section 101 of the Act creates the Veterans Choice Program (“the Program”). Section 101 requires the Secretary to furnish hospital care and medical services to certain eligible veterans through agreements with identified eligible entities or providers. Sec. 101(a)(1)(A), Public Law 113-146, 128 Stat. 1754. Delivery of such care through non-VA health care providers will be at the election of eligible veterans. This interim final rulemaking primarily restates these mandates and prescriptions in a regulatory framework, and provides guidance where Congress' instructions were not clearly executable on the face of the law. Congress directed VA to publish interim final regulations concerning this program within 90 days of enactment. Sec. 101(n), Public Law 113-146, 128 Stat. 1754. This rulemaking complies with that mandate.…

The three primary steps to establishing financial responsibility for insured patients are verifying the patient’s eligibility for indemnity benefits, determining pre-authorize and referral requirement, and determining the main payer if more than one indemnity plan is within effect. There are three factors that ascertain patient benefits eligibility. These factors are coverage might cease on the concluding day if the month within which the employees active full-time service is concluded, such as terminus, furlough, or disablement. The employee might no longer measure up as a member of the group. For exemplar, roughly companies do not furnish benefits for part-time employees. Whenever an insured patient’s policy does not cover a planned service, such situation is talked about with the patient. The medical billing and coding process involves numerous tasks completed by all staff members of a medical facility to provide quality care while protecting the privacy of patients and expediting the payment of services. Ten steps are used to complete this process; pre-registration of patients, establishing financial responsibility for visits, check in of patients, check out of patients, review of coding compliance, a check of billing compliance, preparation and transmittal of claims, monitoring payer adjudication, generating patient statements and handling collections. During pre-registration, HIPAA policies are reviewed and signed by the patient to inform him or her of their rights and responsibilities; therefore, informing the patient of…
