Update 11.10.17: Lyle continues to experience a static level of anxiety that he is able to manage during times when there are no other stressors in his life. When he experiences a disruption in his daily living schedule, he turns to unhealthy means to self-sooth and find relief, primarily drugs and alcohol.
(B) Lyle is struggling in his current living environment as he feels it is an unsafe neighborhood with negative influences. He wants to move elsewhere to remove himself from others who support drug culture. Update 11.10.17: Lyle, as of this report, residence has not changed, and he continues to struggle with neighbors and the drug culture in the area. He has not been receptive to strategies and ideas on how to mitigate some of the things he can control to make his living situation better.
(C) Lyle is on the brink of losing his job as a dishwasher at an assisted living facility where he has been employed for several years. He has had anger outbursts on the job and his attendance has been poor due to drug related insomnia. Update 11.10.17: Lyle is no longer employed. He reports that he was let go from his job due to absenteeism and being under the influence while working. Skills will be shared with those on Lyle’s team so that they can be applied across multiple domains in his life. Plans will be developed that target the presenting problems listed above. These plans include exploring the root cause of Lyle’s anxiety, learning of problem solving skills, building his social support network, creating safe boundaries that he can enforce, dealing with past trauma, becoming more heavily involved in vocational services so that his employment is not impacted, finding resources to address his drug addiction, referral to psychiatric care for mediation to decease anxious symptoms, and learning how to live independently and safely.
