The elderly patient requires specific nursing care; especially those that had a major illness or surgery. In this essay I am going to discuss several safety issues of an older adult client after surgery and the importance of facilitating the appropriate plan of care. In the case study “Elder care case study” the client Mr. Trosack is being discharged after undergoing total hip replacement surgery. As the nurse case manager it is my duty to discuss a plan of care, assess the client’s situation, manage all aspects of the patient care and services, monitor and determine eligibility for treatments. A case manager must also conduct research analyze data coordinate patient care with the entire interdisciplinary team and family to ensure optimum outcomes of the patient needs and goals. When developing the plan of care for the elderly it is essential to identify key issues and concerns. With the use of the nursing process the nurse manager is able to assess, diagnosis, implement, and evaluate the patient’s expected goals. In the case study Mr. Trosack faced many healthcare issues (hypertension, diabetes, obesity, hip fracture), and /psychological factors (social isolation and depression). I would explain that Mr. Trosack requires a Home health aide and nursing services to help reinforce teaching on maintaining a clean and safe environment. These services would provide Mr. Trosack with a nurse and home health aide to help with appropriate medication administration, to help encourage adequate meals and healthy choices, and to make sure the client complies with rehabilitation homecare services such as home physical therapy and occupational
