Case Study On Sebastian Carbello
Sebastian Carbello known as Seb is a 45-year-old Hispanic man who is an Australian citizen. Having a graduate level in law, Seb is a corporate lawyer in Brisbane Queensland. His work life is highly stressful; as it requires time and toll on his daily life outside the office, as well pressure to maintain a professional image. Carbello is divorced with two children and pays child support to assist his ex-wife for their children. As a corporate lawyer, Sebastian is a high-income earner who obtains private health care, household and car insurance.

Recently, Carbello has been experiencing angina (chest pain) for a few months. His health condition of angina and overweightness increases the risk of Carbello in obtaining coronary heart disease,

It is then Sebastian becomes diagnosed with coronary heart disease. The local general practitioner develops a Chronic Disease Management Plan since he is eligible due to his chronic medical condition, allowing Sebastian to receive a Medicare rebate of a maximum of five services (per calendar year) that are bulk-billed (if the provider of the service accepts the Medicare rebate) due to ultrasound results showing symptoms of CHD that could potentially be present in the past six months of Carbello’s life (Department of Health, 2014). Further advice given by the practitioner to manage the chronic disease was to seek a dietician as soon as possible to discuss alternative dietary plans, as well encouraged to increase his weekly physical activity to at least 20-30 minutes five days a week to help and maintain the disease (Government of Western Australia, 2015). Carbello proceeds to a clinical dietician, which is compensated by the Chronic Disease Management Plan through the Medicare rebate. During the check-up, results show Sebastian’s change in lifestyle has not improved. Due to the lack of improvement in his health, his practitioner then advises and referred Sebastian to a cardiologist who specialises in the field of coronary heart disease this service is bulk-billed, leaving all expenses covered by his management plan (Medicare Rebate) (Commonwealth of Australia, 2017). Visiting the cardiologist in regards to his health problem, the cardiologist came into contact with his local general practitioner to discuss the prescription of blood thinner to Sebastian as it will help reduce and prevent future and present clogging within his arteries. This prescription, Warfarin (blood thinner tablets) is on the Pharmaceutical Benefit Scheme (PBS), which enables Sebastian to purchase the medication at a cheaper price (Department of Health, 2017).
