The steps that I will be including in an assessment category is:

a) Study the symptoms explained by Darren Ardolino. Indigestion is caused by both excessive drinking and smoking. But he does not drink so we can make an assumption that the main cause of his indigestion is his excessive smoking habit.
b) Take a several blood test to identify the major cause of the indigestion, to detect the level of acidity in blood.
c) Take an X-ray of his stomach and small intestines to see if any cancerous cells are forming in his stomach.
d) Endoscopy should also be conducted in order to visualise the functioning of his stomach and other organs and to see what is the reason behind the failure of the medication.
e) Check the medical history and health background

The common changes seen with advancing age are loss of hearing capacity, loss of vision, fragile bod, decrease in the immunity of the body, loss of sense of smell and taste, etc. In conclusion when the person reaches to the elderly age he/ she loses his sense of smell, taste, vision and his hearing capabilities. Their skins become loose and gets easily injured, their muscles are weak, their bones are weak. They tremble while working do they will need a walking stick.
The diseases such as burning mouth syndrome, central auditory processing disorder, cataract, muscular degenerate etc. plays an important role in the sensory loss of taste, smell, hearing, and vision respectively. The elderly therefore becomes dependent with their growing age. They are unable to walk, see, hear, and taste.
Darren is also concern about his father, John, who is having sensory loss. His father is 72 and he is very much aware of the impact of old age through his father present health conditions.

It is a medical history of the patient from the day he signed in to the day he leaves the hospital or nursing home. There are certain standards and policies and procedures for documentation of the patients medical history.

Darren is a patient who is having a problem of indigestion, he has taken gaviscon as medication yet the stomach is troubling him. He fears his stomach ache may result to stomach cancer due to his family background of stomach cancer. His father have been recently operated with stomach cancer. After the health assessment of Darren and undergoing several test to determine his health issues, he was requested to stay in the hospital for a period of one month. The reason was to detect the success of the health lifestyle implemented after studying his health issues. He was addicted smoking which was the trigger for his indigestion problem. So he was requested to join the rehab. He was prescribed with the same medication- gaviscon which was the strong drug for maintaining the acidity in the stomach, preventing ulcers. He was ask to follow a routine which included sleeping on time and waking in time, yoga to help with indigestion and taking his food on proper timing. Keeping himself busy with recreational activities to avoid him from smoking, and adequate intake of

    Physical changes start to occur or continue to occur from before being in the facility. The ability to move easily is decreasing, and they may start depending on assistant moving devices such as walkers. They also may need complete help moving around so a wheelchair would be in order. They also may not be as physically capable of taking care of themselves anymore. They may need assistance getting out of bed, showering, and putting clothes on. They also start to show signs of aging such as hair turning gray, loss of elasticity in skin causing wrinkles, height declines, there may be a decrease in muscle tone, and hearing loss may be present. The movement of food through the digestive track slows down because the stomach is not able to produce a great amount of acid to break down food as it did before. They also may start to experience some eye issues such as glaucoma and cataracts. Cognitive changes may be a normal sign of aging however, most of the time they remain stable. Severe changes in cognitive ability may result in dementia. Depression is very common with this age group. Depression may be caused by factors such as isolation, irregular sleep patterns, and loss of appetite. Delirium is also common with this age group, and may be characterized by such thinks like memory loss and confusion. Psychosocial development is a little different in this age group compared to other age groups. This is the age most people retire from their jobs however, a good amount still do work. Living arrangements may change from being able to live by themselves to needing some assistance so they are moved into a nursing home or other facility that help provide care. Health problems could also determine where the person lives. Older adults also like to be include in social events and community involvement opportunities.
