Western Region Community Health Centre has collaborated with Doutta Galla Community Health and North Yarra Community health; now the three groups operate under the umbrella of Cohealth. In turn Cohealth has become one of the largest community health organisations in Australia. Cohealth provides a comprehensive range of services in mental health, oral health, alcohol and drugs, medical, aged care and family violence (Cohealth 2016).
My role as a social worker within Cohealth was to provide initial intake assessments of the client’s needs and the possible risks within their environment that may impact on the client or others. Within the assessment I was to work with the client to identify their needs, goals and accordingly determine what actions or steps need to be taken to eventuate these goals and reduce their dominant issues.

In working with Fatima my primary goal was to provide a non-judgemental, anti oppressive and culturally sensitive environment where she could feel supported and able to express her needs and concerns. Thereupon I aimed to work collaboratively with Fatima in the initial meeting to plan how best she can achieve her desired outcomes.
