An important part of the role of the TB service is to promote awareness of TB and ensuring that all suspected cases of TB are promptly referred for investigations, diagnosis, treatment, support to control the spread of TB.
The most common pathway by which patients can access the TB service includes referrals from GP surgeries, accident and emergency, laboratory microbiology, hospital wards and other hospital specialties such as HIV (Gum clinic), rheumatology and paediatrics. TB services have one designated referral number, fax, email address and contact address. The care pathway for patients who show signs and symptoms of TB will require a number of investigations, which will include a Mantoux Skin test, when a small amount of Purified Protein Derivative (PPD) is injected into the arm and the results will be read 48/72 hours later, a chest x-ray, sputum samples or a bronchoscopy. Once these investigations have been completed and if the results are positive for active TB, the patient will be commenced on TB treatment. This treatment will be for a minimum six months to two years depending on the type of TB

This paper will be addressing a clinical case study from the writer's current experience that illustrates collaborative nursing practice. Effective communication affects all aspects of working in a care setting. One aspect would be with the client where, with effective communication, you could establish what the client’s needs are and agree the best way of meeting those needs. One is also able to empathize with the client’s thoughts and feelings and challenging situations can be interpreted. Another side is with the relatives and friends of a client where support could be provided. A different aspect is with colleagues where new information affecting the client can be passed on such as medication, injuries; also information about rota changes or training to be undergone. In addition, effective communication is required when there is a need to discuss with other health professionals about a client or researching information on an illness. Without effective communication provision of a high standard of holistic care and support as well as performing safe clinical practice would not be able to happen.…

I also think that one to one communication between colleagues is extremely important in Health and Social care because the employees need to be fully aware of their patient’s needs. If they don’t have good communication with each other they could end up giving their patients the wrong treatment. For example if the staff hand their patient over to another member of staff after their shift without telling or writing down that they have already…

Inter-professional collaborations are invaluable relationships between healthcare professionals that are formed with the sole purpose of providing optimum help for a service user (Hornby and Atkins, 2000). Goodman and Clemow (2010) explain that collaborative practice is actually based around two ideas. The first is that nurses need to be able to work with other professionals, and the second is that they need to be able to work with ‘people’. They go on to explain that the concept of collaborative practice is based on the idea that excellent patient care relies on the expertise of a number of care providers. Day (2006) points out that actually defining what collaborative working is, is very complex, and that in order to understand what exactly collaborative working means within the context of nursing, an understanding of what collaboration is, is required, along with the ability to apply this understanding to a number of other terms which are often used both interchangeably and synonymously with this one in literature. According to Day, collaboration can be defined as individuals or agencies working together in order to achieve something that neither can achieve alone. This, however, is not to be confused with the meaning of inter-professional working, which is defined as professionals from different disciplines working between and among each other for the mutual benefit of those involved. Understanding that the two phrases are in fact different in nature allows us to have a more comprehensive understanding of what it means to use the term inter-professional collaborative practice, and this can be defined as professionals from different disciplines working together towards a common goal in order to achieve something that neither can achieve alone.…

In our ever changing healthcare system the need for collaboration is more needed than ever. Collaboration as defined on Wikipedia “is a process defined by the recursive interaction of knowledge and mutual learning between two or more people who are working together, in an intellectual endeavor, toward a common goal” (Wikipedia, 2007). Collaboration is a complex process that requires knowledge sharing and joint responsibility for patient care. Sometimes it occurs within long-term relationships between health professionals. On other occasions, collaboration may involve short encounters. In these settings, there is no second chance to collaborate effectively. The challenge then, is to make the most of all interactions in order to use the best knowledge and abilities of all the health team and produce positive patient outcomes.
