Introduction:
Colonic ischemia is the most common form of gut ischemia, frequently affecting older population. Colonic ischemia may be more prevalent in women. The diagnosis and treatment of colonic ischemia can be challenging since it is prevalent in patients who are debilitated and have multiple medical problems. Here we present an interesting case of recurrent ischemic colitis leading to colonic stricture in an elderly female with multiple medical problems.

She had shortness of breath, chest pain and palpitations and light headedness and reduced exercise tolerance. She had previous admission for the same problem and CTA abdomen was suspicious for ischemia of hepatic flexure. Vitals were stable on admission. Rectal examination revealed brown stool, guaiac was positive. Hemoglobin was 6.7 and coagulation panel was within normal limits on admission and she received blood transfusions.
