Assessment
It is crucial to have a complete assessment about this community to determine their needs ensuring positive intervention and health promotions are put in place to improve the community health outcome.
Initial concerns for this community would include the identification of the health risks associated with smoking and secondhand smoke and dietary needs of the community. Secondary prevention concerns that would need to be address include therapeutic treatment of the signs and symptoms of respiratory infections, fatigue, and anemia. The tertiary level of health promotion would consist of returning this community to a positive health status through the use of dietary consults, improved access to healthy affordable foods and medical treatment for medical diagnosis. It is critical to review and assess the health determinants of this community to undertake appropriate interventions for this targeted population. Assessing the determinates of health that would need to be focused on would include income and social status, social support, social and physical environment, personal health practice and coping skills, health child development and health services (Public Health Agency of Canada,
2011).
Health Promotions and Strategies Population health promotion is the process used to enhance the health quality of the population through the practice of assessing and taking action to change features of the social determinants in return, creating positive change behaviors in health as well as encouraging and promoting community participation (Stamler, Yiu, & Dosani, 2015). Community participation is encouraged when in the phases of planning, and when making decisions regarding personal health concerns ( CARNA Guidelines 2008, P.3). Collaboration with the families and individuals of this community is needed to assess and develop a complete understanding of their perspective of the situation and the ability to initiate meaningful change. According to The Ottawa Charter for Health Promotion, including community members promotes changing social, environmental, and economic conditions to alleviate their impact on health (Stamler et al, 2015, p 137). Strategies to promote health promotions would include, thorough community assessment, establishing and stabilizing health policies, strengthening the community through provision of a supportive learning environment, educational sessions, developing skills and abilities, enabling the use of government funded programs and local health care services (Stamler et al, 2015). Fostering a positive environment for change and placing the onus back on the community can help to initiate positive change.
