After analysing the scenario, I have identified six goals, three of which are immediate and the other three are time critical but not as crucial as the most immediate. The first priority is that Janet (the service user), was found in a bed soaked with urine, the reason for this is that her catheter came out during the night, and her husband (Bob) wouldnt use the hoist to change her sheets and wash her. I feel this to be of immediate priority, as Janet will not only be uncomfortable, but it will have a negative impact on her skin integrity. This adds to the risk factors already contributed to Janets skin integrity being compromised. Other risk factors for poor skin integrity include inadequate oral nutrition, dehydration, lack of sensory perception, decreased mobility or medical conditions that affect circulation. (Hart 2013)
