D-The patient arrived late to the Women's Group and stated that she had gotten the time confused from 10:30 am instead of 09:30am. The patient reports she is not stable at her current dose because she is experiencing cravings. She reports using 2 bags of heroin by inhalation, no cocaine last night. This writer completed the dose change request form as the patient requested to increase her dose by 5 mgs. Furthermore, this writer discussed with the patient about her triggers and addressing alternatives to refrain from illicit drugs. The patient reports that she missed group again at Wheeler Clinic as part of her court requirement and does not know what will happen next. The patient mentioned that she is going to the courthouse today, hoping she will not get arrested and wants a second chance of doing the group again. This writer addressed her barriers to getting to Wheeler Clinic on time such as leaving her home early to get those by …show more content…
This writer discussed with the patient about alcohol while at a methadone clinic can cause a significant effect with her recovery process.

PCP-
The patient hasn't yet located a PCP. This writer again, provided the patient with a listing of PCP in her area and gave her deadline by 03/03/2016 to provide a PCP. This writer will monitor the patients UDS and daily dosing. Provide positive feedback for negative UDS results.

A-Based on this writer's assessment, the patient appears to be alert and oriented. There was no evidence of SI/HI. At this time the patients appear to be under the preparation stage of change as she wants to change behavior in her recovery process, but yet did not resolve her ambivalence behavior for her ongoing use of illicit drugs. The patient needs more convincing and encouragement for her recovery.

P-The next appointment is scheduled for 03/03/2016 at 9am. The patient will update this writer on the status of her PCP

    Critical Incident Analysis INTRODUCTION A critical incident analysis is one in which human behaviour is observed and data collected to provide a practical approach to solving practical problems as well as developing some psychological principles (Flanagan 1954). This critical incident analysis will address some of the important issues around the care that Sarah received as identified in the initial incident (Appendix 1). The focus will be on confidentiality as this was the basis of the incident. The hypothesis to be determined is to ascertain whether Sarah was entitled to confidentiality indicating the General Practitioner was wrong to send out the letter to her parents. Issues will also be discussed in relation to Sarah’s care to consider what steps could have been done to improve the overall care that she received. This will be undertaken using a wide range of literature which will be critically appraised using appropriate research frameworks to determine the quality and reliability of evidence used. This will include data that is qualitative and quantitative as well as systematic reviews. This should form a conclusion which should prove or disprove the hypothesis. DISCUSSION Confidentiality is a very complex issue especially with adolescents and there appears to be no clear guidelines for Health Care Professionals to follow. The NMC code of conduct (2008) stipulates that people’s right to confidentiality must be respected which therefore suggests that this applies to both adults and children. Burnard and Chapman (2003) recognise the necessity of confidentiality and stipulate that this should be honoured to adhere to the NMC Code of conduct. It is a concept which is based on the Hippocratic Oath which states that anything heard in secret should be kept silent. One of the important factors of confidentiality that is highlighted is trust.
