Minimum information on this ID band must include the patient’s first and last names, (middle initial is desirable) and unique identification number, also referred to as hospital number, medical record number or case number. Gender, age, and patient location are desirable but not mandatory. The phlebotomist must compare and confirm this information with the label on the sample tube. The label on the sample drawn for crossmatching must include the patient’s first and last names, identification number, and also the date and time the sample was drawn, and a means to identify the phlebotomist. Many institutions choose to have the phlebotomist sign or at least initial the sample, others have the phlebotomist sign the orders. Identifying the phlebotomist will help resolve any problems in patient identification, if there is reason to believe the incorrect patient was
