The presentation will provide evidence from the internal evaluation of an applied research collaboration funded by the National Institute for Health Services Research in the UK. The collaboration called a CLAHRC ( Collaboration in Leadership in Applied Health Research and Care) is a large grant ( £20mil) and includes five universities and 18 care provider organisations . When considering what type of methods of disseminating evidence would work best in my environment I took into account the size of my facility and the culture. The facility I work at is a small local assisted living that provides care for 45 patients. Since the facility is small in size it only employs eight nurses, including the nurse manager and administrator; along with fifteen certified nurses aids. The size of the facility helps create a family and strong team environment not just for the staff but also the patients. The opportunity to demonstrate delivering optimal care using an evidenced practice for developing a plan that entailed prescribing medications, conducting patient education, identifying additional tests, making referrals, and identify follow up needs. The trial study established the ability of student groups to recognise and act on care need in a susceptible patient society that is well suited to an interprofessional model care. After a few short months of working, I knew that nursing was the career path I wanted to pursue. The patients I see day to day are remarkable. Although there are several patients I admire, there is one young girl that caught my attention from the day I met her. She has a rare complex medical condition which includes extensive therapy and several ongoing surgeries. But when you meet her, you would never know. She is overwhelmingly strong and passionate about achieving her goals and aspirations in life. I too, believe in her and want to help her accomplish her dreams. Through her I was able to realize my true passion for advanced practice nursing and my desire to help others through their healing process. As much as it can break one’s heart to see someone suffering, it also fills one’s heart to be there for the patient and their family. Although it may seem as though a nurse’s help goes unnoticed, for me, it is the satisfactory feeling of letting patients know they are not alone and being there for them through it all. That sense of joy is just one aspect of what makes me want to become a PNP.
