Leadership and Management in the Changing Health Care Environment

The future of nursing is evolving at a rapid rate due to a changing health care environment. Nursing practice and leadership must adapt to these changes through ingenuity and advanced practice professional roles. “With an estimated 32 million newly insured Americans set to have health care coverage by 2014, and a predicted shortfall of 40,000 primary-care physicians by 2020, market forces have virtually dictated that more frontline providers are essential to the sustainability of our nation 's health care system” (Brocato, 2010, para. 1). In order to mend the projected shortfall of primary-care physicians, the role of nurse practitioner must evolve with increased autonomy with a broader spectrum of ditties and responsibilities. This change must occur because more physicians are moving into specialized fields and the need for primary care physicians will be supplemented by the nurse practitioner.
The key stakeholders involved in the changing role of the nurse practitioner are those needing primary healthcare, now and in the future. “According to the American Association of Colleges of Nursing, about two-thirds of the more than 140,000 nurse practitioners in the United States practice …show more content…
in adult or family care” (Raines, 2011, para. 17). In most states nurse practitioners are reimbursed by insurance companies, Medicaid, and Medicare. The expanding role of the nurse practitioner will help fill the void and lack of primary care physicians. Nurse practitioners are typically less expensive than primary care physicians. This will benefit the patient with affordable healthcare, and influence the healthcare budget of Medicare and Medicaid. As the demand for healthcare resources skyrocket out of control, expanding the role of the nurse practitioner will benefit society in many ways.
The nurse practitioner faces many challenges and must ban together in strength and organization to implement strategies that enhance their public relations as perceived by society and other medical professionals. The nurse practitioners scope of practice varies from state to state. These inconsistencies cause confusion thus diminishing their professional acceptance. “These discrepancies are contrived from strong efforts on the part of some physician groups that perpetuate the misconception that APN care is substandard and hazardous to the public’s wellbeing”(Nurse Future, 2012, para. 5). Many physicians see the nurse practitioner as a threat to their lively hood and professional respect. Nurse practitioners must gain political strength and find common ground with the AMA (American Medical Association) in finding solutions that plague the health care industry. Nurse practitioners and physicians must work together, because together they can face the uncertain and challenges of societies healthcare needs.
Implementing strategies to diminish the barriers faced by nurse practitioners is a work in progress.
One of the greatest barriers faced by the nurse practitioner is inconsistency of state laws influencing their scope of practice. Until nurse practitioners are held at the same standard in all states, the seriousness of their professional role cannot be revered. “Multiple research studies indicate that primary care services and management of acute illnesses and chronic diseases can be effectively provided by an APN, with positive patient outcomes that are reflective of a physicians’ care” (Nurse Future, 2012, para.
12).
In time the role of the nurse practitioner will evolve as the need for primary health care increases. In order for change to take place the nurse practitioners must become politically motivated. Currently the AMA hastens the advancement of practice of the nurse practitioner. The use of nurse practitioners is cost effective, and with minimal resource with increased demand in the healthcare arena, utilizing nurse practitioners makes sense. “A cost data analysis from the Rand Corporation found that cost per patient visit was 20% to 35% lower when utilizing an APN or PA than with traditional physician visits, resulting in estimated statewide savings of approximately $4.2 to $8.4 billion from 2010 to 2020.5” (Nurse Future, 2012, para. 13). In time nurse practitioners will prove themselves as key healthcare providers working in harmony with physicians.
