A woman Gravida 1 para 0 was admitted to the midwifery led unit in hospital in labour at 37+3 weeks gestation. Ms Amy Dent presented herself with tightenings and on examining her vaginally, the midwife found her to be 4cm dilated. She began using Entonox as pain relief since she could not cope with the pain naturally. After an hour and a half, Amy expressed her wishes to enter the pool and on examining her vaginally the midwife found her to be 6-7 cm dilated. She allowed her to enter the pool and Amy progressed quickly and after an hour she delivered a baby boy.
Following the delivery, the midwife discussed Amy’s options for the delivery of the placenta and she opted for a physiological third stage delivery. Slight blood loss was noticed in the pool and the midwife advised her to push in order to deliver the placenta by maternal effort.
After twenty minutes there was still no placental delivery and the midwife re-discussed the third stage management with Amy and she chose to be managed actively.. Therefore the midwife administered 10IU of syntometrine via an intramuscular injection into Amy’s
