Client’s Profile
Madam Y.A.L is a 70-year-old Chinese who speaks Hokkian and Mandarin. She is 1.58m tall and weighs 66 Kg. She is living alone in a three room flat in the Northern part of Singapore over 32 years. She has 3 children, two older sons and one younger daughter who are all married. Her children have one son each. Her daughter lives nearby and brings her to eat food outside together once a week. Client loves shopping and cooking. Client is a Christian and usually goes to church every Sunday to attend church service. Both her sons always go over to have dinner with her. She is also looking after her two grandsons after their childcare session till both her sons come and pick them up from her house.
1. HEALTH PERCEPTION AND HEALTH MAINTAINANCE
A. Review of Client’s Health Perception
Client verbalized that she has been worried about own health since 2010 when diagnosed with diabetes. She rates her health state 6 out of 10 (1-worst, 10-best). She is not aware of healthy lifestyle. She likes to eat oily food but she is not willing to do exercise. She claimed that she had a lot of housework to do and often felt tired after that. She denies of smoking and using alcohol.
B. Review of past Health Status
Past illnesses/Hospitalisations
1. Type 2 DM and hyperlipidemia for 3 years- under long term medications. Both parents passed away due to heart disease from effect of high blood and cholesterol level.
2. Cataract removal with intraocular implant done in right eye in 2011.
3. Menopausal in 2009.
4. Fracture over femur of left hip due to a fall in 2010 and surgery was done.
Client’s husband is also a Chinese. He is a smoker that smokes around 10 sticks of cigarette every day since young. The most recent body check done was in 2011 and nothing significant was reported except was informed by family doctor that his cholesterol is on the high side of the normal range and required him to monitor his diet.
