Stroke also known as a cerebral vascular accident (CVA) is the disruption of blood supply which damages the brain and causes sudden onset of neurological deficits (Lundy-Eckman, 2013).

Strokes are classified by two broad categories, hemorrhage and ischemia which differ by the incident of the cerebral blood flow. Hemorrhage stroke is characterized by a rupture or leaking of a cerebral artery causing damage to the surrounding brain tissue (Caplan, 2016). Hemorrhage can be further classified into area affected such as intracerebral or subarachnoid (Caplan, 2016). The most common form of stroke is Ischemic, which it is the occlusion of a cerebral artery that causes damage to the brain tissue depriving it of adequate amount of oxygen and nutrients supplied by the affected vessel (Caplan, 2016). The OT has a clue to the client’s rehabilitation length of time, as each subtype has their own signs and symptoms, pace for recovery, and characteristic course (Caplan, 2016). The OT had a realistic expectation of healing and goal setting, and possible complications, such as aphasia when communicating with the client. The client had experienced a severe left Middle Cerebral Artery (MCA) Ischemic Stoke which is the most common type of stroke (Lundy-Eckman, 2013). Since MCA supplies the language area of the brain the OT knew there would be communication issues and planned for tools to communicate with the client. The OT knew they needed realistic goals, and had to plan for long term, knowing the client would not improve much beyond what was already accomplished.
