Hand-offs occurs when patient care is transferred from one provider to another provider. It is important that the information about patient care is timely and accurate. In clinical settings, hand-offs have had some challenges. In efforts to address the challenges information technology has been designed to make a smoother transition.

Discharge worksheets can contain electronic data from patient summaries. The discharge summary can include information which post discharge providers can use to treat a patient. The summary can be faxed automatically to other clinical personnel. A final report of the patient can be automatically complied to include medication lists and instructions for the patient.
Process-oriented information tools like computerized rounding and sign-out tools are good for improving communication during multidisciplinary rounds. Patient information from electronic health records can be included in the patient data. It helps the providers to make more informed decisions while providing health care services. These tools include information such as discharge needs, check-off lists, and daily goals.
Electronic information such as decision support tools is useful in clinical settings. Reference materials, protocols, and guidelines are provided in decision based scenarios. Providers are able to talk about important variations from a clinical pathway. There are also graphic displays that clinicians can use to see laboratory results on smart phones and/or mobile
