What are some common nursing diagnoses for patients with HTN? (2.5 points) "Knowledge Deficit Related to lack of information about the disease” "Risk for Decreased Cardiac Tissue Perfusion" “Headache related to increase Cerebral Vascular Pressure” "Risk for Decreased Cardiac Output”

After the diagnosis of HTN is confirmed with initial studies, what further evaluations are necessary? (2.5 points)

Why is the registered dietitian important in the multidisciplinary team? What is the primary role? (2.5 points)
Dietitians are very important in the Nursing Process because they help to determine if the patients are malnourished or at risk for malnutrition.
Dietitians may obtain much of the preliminary information about the patient from the nursing history and physical, such as height and weight; skin integrity; usual diet prior to admission; difficulty to chewing, swallowing, or self-feeding; chief complaint; medication, supplements and aver the counter drug used prior to admission; and living situation. Once the dietitians gather the information and interview the patient; they are able to make a nutritional diagnostic to provide with an appropriate diet for each patient, based in their current conditions, in this case of Hypertension the dietitian may have to advice a Cardiac diet with is low in lipids.
State the category and purpose of the medications ordered. (2.5 points)
Furosomide ( Lasix) Loop diuretic to decrease body fliud and improved blood pressure
Captopril ( Capoten) Angiotensive-Converting Enzyme Inhibitor. a type of drug to treat Hypertension, this drug blocks the the effect of angiotensive II, allowing the body to rid of fluid, Na, Cl, K, and uric acid and improve Hypertension
Hydrochlorothiazide ( Hydrodiuril) a Thiazide diuretic to treat hypertension. This drug increase water excretion by increasing GFR or Sodium reabsorption from the tubules
