Dr. Williams and Class,
From day one of nursing 101, the Professor told the class that “nursing is a continuous lifelong learning process.” In any organization, transformation of change must be done through effective communicating with all stakeholders, and this is the first step in the planning phase (Chamberlain College of Nursing 2016). Today nurses are better prepared to taken on these challenges and overcome barriers to change.
This project change in the Intensive Care Units is important to start the phases for implementation of:
1. Reinforcement and adhering to discharge instructions (such as: difficulty in breathing, fluid management, sodium/salt restrictions, weigh daily, medication, in heart
failure or cardiomyopathy (HF/CMO) patients will decrease the readmission rates.
2. Families and caregivers will be presented with credible websites, brochures, a smoking cessation packet on ways to quit (if applicable); they will be encouraged to ask for further clarification on subject content.
3. Availability of team members during the implementation process is important to answer question and trouble shoot, this can boost staff confidence ( Dearholt & Dang, 2012). In this week’s lesson (Chamberlain College of Nursing, 2016) stated that “Completion of a successful evidence-based practice project can lead to the adoption of an effective innovation by an organization.” The progress on the decreasing readmission rate depends on the timeframe an individual spend in each phases. (Bridges, 1991) Transition Model gave a clear description of what happened in each phases. Clearly, his article reminded us that whether we accepted or dejected change, it is inevitable that at a certain point throughout our lives people usually ease themselves into supporting change.
