MS-DRG Medicare severity diagnosis-related group or MS-DRG is Medicare refinement to the diagnosis-related group (DRG) classification system, which allows for payment to be more closely aligned with resource. The Medicare Severity-DRG (MS-DRG) is the most commonly used DRG system, because it governs the ever growing ranks of Medicare patients (Bushnell, 2013). MS-DRG has a payment rate called a weight (Casto, 2013). Higher weights are associated with groups in which patients require more resources for care and treatment. Higher resource consumption is related to higher intensity of services due to the severity of illness or the type of services needed for the care and treatment, such as

In the article Medicare Under Observation, it is mentioned how unfair and ineffective the Affordable Care Act is for senior citizens in the United States. The article emphasizes on a letter written by a Medicare beneficiary who recently had gone to the emergency room to get treatment for a urinary tract infection and ended up having to stay for hours only to be told he was not going to be able to go home and he was being admitted into the hospital for observation which he claims that under the Affordable Care Act law Medicare will not pay anything on. This left him very puzzled and wondering how come he needed to stay over a period of two to three days. This being the case, he would have to be responsible for paying this medical bill in full…

When physicians provide a service in a facility such as a hospital, the total RVU is lower due to the fact that they do not incur the full practice expense associated with providing that service; such expenses are the cost of having full staff, equipment, space, or supply costs. Hence, Medicare reduces the payment based on the location of service. On the other hand, the most common non-facility location is the physician’s office…

Currently, hospitals are paid a flat fee per hospital case by the federal Medicare program, using the Inpatient Prospective Payment System (IPPS). The prospective payment price, also referred to as the DRG (Diagnosis Related Groups) payment, covers all hospital costs for treating the patient during a specific inpatient stay, including the costs of all devices that are used. CMS adjusts DRG payments annually to reflect changes in hospital costs and changes in technology. This fee is paid to the hospital based on the patient’s symptoms, age, sex, discharge status, and the presence of complications, but does not account for length of stay or how many hospital services are actually used. (Ellis, 2011) Over time, the attempt has been to keep these rates close to the average cost of providing the services per case, although many hospitals claim that often the case payments they receive are below their own full costs. (Reinhardt, 2009)…

As for the outpatient coding, the first listed diagnostic code indicates the reason for the encounter. In conclusion, the outpatient coding summarizes all diagnoses and typically includes a single procedure whereas inpatient coding requires daily coding of each service on each day of hospitalization.…

Two of the main factors that have led to decrease in hospital inpatient days and a growth in ambulatory services are Medicare reimbursement and cost-saving efforts of managed care. Medicare instituted PPS for reimbursing hospitals in the mid 1980s. PPS repayment based on DRGs gives fixed case-based payment to hospitals. Therefore, hospitals have a strong reason to decrease the inpatient length of stay and continue in an outpatient setting. Outpatient area has fewer payment restrictions. Managed care adopted a cost-containment strategy, which also stresses lower inpatient utilization,…

According to (“Patient Advocate Foundation” 2012), “care can be provided in a larger geographic service area than would be possible with only one physician group. Problem: Last year federal centers for Medicare and Medicaid Services announced that they would no longer reimburse hospitals for treatment of new pressure sores in Medicare patients. The ruling, known as the Inpatient Prospective Payment System (IPPS) final rule, adopts a new Medical Severity Diagnoses Related Group (MS-DRG) classification system that expands the current number of DRGs from 538-745, with weighing factors that will be phased in over a 2 year period.…

Thank you both for your comments. As you have both mentioned the Medicare rule for observation is very complex and confusing. The intention of the rule is to set clinical criteria for observation, and rapidly discharge the patient in twenty-four hours. However, this does not always happen. Some observation patients can stay up to ninety-six hours before being discharged or switched to observation status. One of the problems is individual health care organization interprets the Medicare rule differently. Then if the patient had a managed care Medicare carrier, the rules may be different. This makes observation status for an organization nightmare to manage (Hockenberry, Parlato, & Ross, 2014). What I find even worse, observation increases the…
