Richardson Molding (packing flag kits and placing washers on screw boards), Central Electric (stuffing letters in envelopes, wetting and sealing flaps on envelopes, and applying mail labels), Central Office Supplies (labeling water bottles), Anthony's Syrup (forming boxes and labeling and packing containers), Star Herald (collating inserts and applying labels to newspapers). His average production rate was 41%. He also did janitorial work at AbilityWorks, two churches, and an office building. The instructor noted the following limitations to employment: fluctuations in daily work levels, and poor physical stamina (sedentary work only). Parks is often frustrated with his health and lack of employment. Additionally, during the review staffing, Mr. Parks verbalized that he is very frustrated because he wanted to work. Mr. Park explained that he has not made any progress since he started receiving rehabilitation services. However, Counselor redirected Mr. Parks, providing guidance in which she had him compared his health today before he began receiving rehabilitation services. Counselor verbalized to Mr. Parks that he had made progress with his health issues. Counselor explained to Mr. Parks that he walked through the door of MDRS needing accommodations to walk, which now he no longer needs.
Mr. Parks continued to state that he wanted to work. Staff explained efforts were made in seeking employment. Staff to discuss possible community based tryouts; however, Facility Manager and Evaluator must complete a job site evaluation to ensure adequate supervision, safety measures are place and operating effectively, and worksite accommodations are

Mr. Parks stated he has pushed through pain crisis daily; however, his overall goal is to become employed. Mr. Parks stated that he has ongoing pain daily all over his body. Mr. Parks that he cannot afford to seek treatment at a pain management clinic. Counselor stated that VR can assist with affordability for treatment. The Counselor impressed upon him, the importance of getting a referral to a pain management clinic. Rudely, Mr. Parks stated he was referred to a pain clinic previously. However, later, he did apologize for the rudeness. Counselor explained to Mr. Parks the importance of contacting his physician for a referral if his physician deemed it is medically needed.
