Necrotizing Fasciitis Case Study
The treatment for necrotising soft tissue infection involves the principles of treatment for any kind of surgical infection: source control, broad spectrum antimicrobial therapy, support, and monitoring .Once the diagnosis of necrotising fasciitis is suspected, immediate aggressive and thorough surgical debridement should be done.The goal of surgical intervention is to remove all necrotic tissue and achieve hemostasis. The wounds are incised and drained.The incisions are made parallel through the discoloured skin down to the fascia parallel to the cutaneous nerves and blood vessels.In necrotising fasciitis the grey necrotic fascia is identified and confirmed by the ease of blunt dissection.The tissue is excised for frozen sections or definitive histology and

    Deep to this area of apparent ulceration there is bright T2-signal present at the dorsalateral aspect of the foot at the level of the mid to distal metatarsals suggesting edema and and/or inflammation in the dorsal soft tissues in that region.     PROCEDURE: The patient was brought into the operating room and placed on the operating table where spinal anesthesia was induced. The right lower extremity was prepped with bednine solution. A newmatic ankle tourniquet was placed over Webril padding above the right ankle joint. The leg was evaluated for proximately two minutes and the tourniquet was raised to 275 millimeters of mercury. An S-shaped curved incision through the right plantar fore foot ulceration was made down to abyss cavity where a moderate amount of sanguinopurulent discharge was noted.
