Regardless of the outcome of an individual “Never Events” are always considered and described as serious incident framework, in addition the identifying the problems solutions are important to improve application of “Never Event” policy and improve patient safety; organizational leaders should make a patient safety program, mentorship, managing financial and human resources which is the most important elements and their performance and strategic planning which aimed to prevent harm while applying patient’s care by applying of “Never Events” policy (Pinnock, 2012).

This policy should be punchy through consultation with stakeholders and offers a useful reference point for boards, clinicians, staff and patients. It does alter the basic principles of the “Never Events” policy and does amend all types of incidents, however, clarify of areas that caused some uncertainty and provide a comprehensive set of frequently asked questions to clarify some of the issues that health care providers and commissioners have raised.
For the purpose of this paper strongly recommend focusing on human’s factors, that all boards consider this refreshed framework and that medical and nursing directors take a lead to ensure that work is taken forward to improve patient safety and eradicate “Never Events” from health
