Neurological System (headaches, head injuries, dizziness, convulsions, tremors, weakness, numbness, tingling, difficulty speaking, difficulty swallowing, etc., medications):
Patient reports stress headaches once a week in the last 3 months. During birth of her daughter 45 years age patient reports she had convulsions. Head was pounding pink, purple, and blue lights. Once on the elevator hand started shaking she remembers nothing else.
Toxemia when pregnant, big baby collar bone broken during birth.
Right hand and bilateral feet tingle. Occurs once a day (patient is a diabetic). Hands wrist and ankles swell when cleaning. Ankles stay swollen. Patient refuses to take medicine, reports doctor wrote her a prescription for metformin but she told the doctor “No, I will control with my diet.” Patient vitamin D level 22, MD sent her to a special place to jump start her Vitamin D level. Patient received 10 pills at a cost of $55, patient reports they made her sick and she only took 3 pills and will not take any pills.
Head and Neck (pain, headaches, head/neck injury, neck pain, lumps/swelling, surgeries on head/neck, medications): Patient reports neck pain that radiates to shoulder rarely occurs last episode 6 months ago.
Eyes (eye pain, blurred vision, history of crossed eyes,

    History of present illness: The patient is a very pleasant 57 year old female, a native of Cuba, being seen for evaluation and treatment for sores her in mouth that she has had for the last 10-12 days. The patient has a long history of severe and debilitating rheumatoid arthritis for which she has had numerous treatments, but over the past ten years she has been treated with methotrexate, quite successfully. Her dosage has varied somewhere between 20 and 25 mg per week. About the beginning of this year, her dosage was decreased from 25 mg to 20 mg, but because of a flare of the rheumatoid arthritis, it was increased to 22.5 mg per week. She has had no problems with the methotrexate as far as she knows; she also took an NSAID about a month ago that was recently discontinued because of the ulcerations in her mouth. About 2 weeks ago, just about the time the stomatitis began, she was placed on an antibiotic for a suspected upper respiratory infection. She does not remember the name of the antibiotic, although she claims she remembers taking this type of medication in the past without any problems.…

    When discussing utilizing the nurse to their full extent, the IOM report touches on how the nurses are able to practice within different scopes, not based on the extent of the education or their training, however it is based more off of which state the nurse practices in. For example, in some states a nurse practitioner is able to write prescriptions without a supervising physician, and other states do not. This is a clear example of the education or training not determining the extent the nurse practices at, yet the states they are governed by making the decision. With the ever-growing population, it is said in the IOM report that it would be more beneficial to have more nurses at an advanced level in outpatient settings. It is in the best interest of the community in my opinion, which we allow advanced practice nurses or APRNs to practice to their full scope, especially when it comes to primary health care. In Las Vegas it takes 4-6 weeks to get in with most physicians, even as an established patient. With that being said, people are choosing to utilize the ER as their primary care provider because they will get care faster. This leads to overcrowding of the ERs. Additional benefits to allowing nurses to practice to their full extent include improved access to the care of the poor and rural areas, respite and palliative care, care coordination for the chronically ill and elderly, nutrition services, and evaluations that compliment care of other physicians and providers.…
