Case Study# 2
1. Heart failure (HF) is a syndrome that involves dysfunction of the cardiac muscle, it occurs with “any of disorders that damage or overwork the heart muscle” (Karch, 2017 p.751). Some of the disorders that may lead to HF are: coronary artery disease, cardiomyopathy, hypertension, and valvular heart disease (Karch, 2017). What ends up happening as a result of these disorders, is that the heart muscle cannot effectively pump blood throughout the vascular system (Karch, 2017). In left-sided heart failure, the “blood backs up into the lungs which leads to pulmonary vessel congestion and fluid leakage into the alveoli and lung tissue” (Karch, 2017 p.752). In right-sided failure, the blood backs up in the venous system, which may lead to liver congestion and edema of the legs and feet (Karch, 2017).

Digoxin “increases intracellular calcium and allows more calcium to enter myocardial cells during depolarization” (Karch, 2017 p.756). The increase in calcium leads to: increased force of contractions, increased cardiac output and renal perfusion, slower heart rate, and decreased conduction velocity through the AV node (Karch, 2017).     Congestive Heart Failure can be brought on by many conditions. These conditions include Coronary Artery Disease, Heart Attack, and lastly Cardiomyopathy. Coronary artery disease can develop in humans after years over time after years of fatty deposits in the vascular system (Arteriosclerosis).     Congestive heart hailure, also known as CHF or heart failure, affects the lives of 5 million Americans each year with 550,000 new cases diagnosed yearly. (Emory healthcare, 2013) CHF is a medical condition in which the heart has become weak and cannot pump enough blood to meet the need for oxygen rich blood required by the vital organs of the body, less blood is pumped out of the heart to the organs and tissues in the body and pressure in the heart increases, it does not mean the heart has stopped working.     Heart failure affects almost 6 million Americans. About 670,000 people are diagnosed with heart failure each year. It is also the leading cause of hospitalization in people older than 65. Congestive heart failure means that the heart is still pumping blood, but at a slower rate than normal, so the pressure in the heart starts to increase as a result. This slower heart rate causes the heart to be unable to pump enough blood to provide the rest of the body with the amount of nutrients and oxygen that it needs. As the pressure increases in the heart, the chambers stretch to hold more blood, or they become stiff and thickened. This compensation mechanism works, but eventually the myocardium (muscle layer of the heart), will weaken and the heart will decrease in its efficiency to pump blood. This results in a reduction of blood supply to the kidneys, which then begin to lose their ability to excrete salt and water. This lessened function of the kidney causes the body to retain more fluid. The fluid build-up then leads to edema or congestion of tissues, hence the name congestive heart failure.…
