This case study is a perfect simulation of an error that is very common during the admission process that is highly preventable. The admission nurse did a great job by asking the daughter of the list of medications that her mother was taking at home. This would have been very beneficial through the medication reconciliation process if the medication list submitted by the daughter was accurate. The admission nurse should have made sure to pass down to the nurse coming on to the next shit, or through a nurses’ note that the daughter needed to bring the medication list in for further verification. The nurse could have also verified Ellie’s medication list with her primary care physician. At the time that Ellie’s daughter was visiting during the first post op, the nurse administering the medication could have questioned the order, considering that the daughter was hesitant on the dose being administered. This is associated with cause and effect because even though we have that one hour before and after window to administer a medication, time management according to patient status is vital. This would also be a great topic to do an in-service on. It would require research or multiple shared scenarios like this one, showing the impact that time management defects in treatment delay can have on patient rehabilitation complications. One intervention that might make an immediate influence is the nurse assigned to a declining patient coming forward to a facilitator or supervisor to say that she has a patient in need of one on one care due to their deteriorating state. Another intervention that can make have a positive long-term effect is promoting team work, simply encouraging staff members to stay in tune with the next co-worker’s assignment.
