When I explained to him my reasons for walking into his office without hesitation he offered me a sit. Mr. Carpenter first explained before I asked him question who he was and what his position was at the facility. When he got done i processed to ask him my first question about Woodlands Mission. Mr Carpenter stated that their vision is that they aspire to improve the health and well-being of their patients through innovative care and the pursuit of excellence. He continued to explain that their mission at Woodlands Rehabilitation and Healthcare Center is Oraianna Health Systems’ patient-focused care model aims to empower their employees to surpass their patients’ needs and expectations with empathy and compassion, dignity and integrity. My next question I asked Mr. carpenter who does Woodland Serve or their target population. He explained that his agency serves the local community which is Clinton, MS and the Jackson-metro area. One thing that stood out to me about the discharge process is that I asked what if the client is not in a suitable environment, he explained that the social worker can only suggest a better environment. If the clients deny then there is nothing that the social worker can do but respect the client’s decision. Proceeding with my next question I asked What are the most critical problems facing the agency’s targeted population. Mr. carpenter explained that the problems that they are facing were people living their facility and living at home alone. I asked him why was that a problem and he explain that things happen when they are home alone. Mr. Carpenter began to explain that older people tend to injury their selves such as falling, and if no one comes to check on them they could eventually die in that same spot.
