Nursing Interventions
As hospitals are moving toward EHRs, the need for nurses to consistently document problems, interventions, and outcomes is crucial. According to Tastan et al. (2014) the use of standardized nursing terminology (SNT), “could improve the consistency, content, and format” (p. 1161), of the communication between nurses, as well as other healthcare providers. The adoption of the Nursing Interventions Classification (NIC) allows for analyzation of specific nursing interventions on patient outcomes. The NIC, according to Cruz et al. (2014), allows for the integration of data related to the unique interventions nurses provide which will in turn support productivity and outcome measurements.

Kari Sand-Jecklin and Jay Sherman, (2014) have stated that their study had been conducted for quantifying the quantitative outcomes of a practice change to blended form a nursing report. In this regard, several advantages of bedside nursing shift report have been identified in the literature. But the published studies failed to adequately quantify the outcomes related with this process change as these studies had either unreported or small sample sizes or were not tested for statistical significance. The method used in the study was that several medical surgical units from a large university hospital implemented a blend of recorded as well as bedside nursing report. The outcomes that would monitor included the satisfaction of the patients and nurses, patient falls medication errors and nursing overtime. It was found in this study that there were statistically significant improvements taking place in post-implementation in the four patient’s survey items particularly impacted by the…

Nursing- Sensitive Indicators are factors that can be used by nurses to directly impact the outcome for the patients they serve (Sauls, 2013). If the nurse had used what was known about nursing-sensitive indicators the outcome would have been positive for the patient and family. Nursing-Sensitive Indicators use structure, process, and outcomes to develop nursing care geared for the patient and directs the nurse to carry out the care needed for that patient. The structure of nursing-sensitive indicators looks at various aspects of nursing such as staffing levels, mix, experience, and education level (Sauls, 2013). Structure looks at the number of Registered Nurses (RN), Licensed Practical Nurses (LPN), and Certified Nursing Assistants (CAN)…

The National Database of Nursing Quality Indicators (NDNQI) established by the American Nurses Association (ANA), provides reports on how care, specifically provided by nurses, affects patient outcomes. Based on these reports, the ANA has developed a national set of nursing sensitive indicators that measure the process of care, the structure of care, and patient- focused outcomes in a health care setting. NDNQI 's aim is to help nurses plan and implement patient safety and quality improvements through evidence-based care (Montalvo, 2007).…

As a result of the introduction of computer technology and the combination of evidence-based practice in nursing; standardization of terminologies has become imperative in the classification of nursing diagnosis, interventions and expected outcomes. The most popular and successful systems are the North American Nursing Diagnosis Association International (NANDA-I), Nursing Outcomes Classification (NOC), and Nursing Intervention Classification (NIC) (de Lima Lopes, de Barros, & Marlene Michel, 2009). This paper aims to provide a brief outline of these standardized terminologies (STs) as they relate to a patient with congestive heart failure (CHF), to identify the elements of NANDA-I, NIC, and NOC, and describe the data, information, knowledge and wisdom (DIKW) used during the process.…

Nursing informatics can best be described “as the integration of data, information and knowledge to support patients and clinicians in decisions across roles and settings, using information structures, process, and technology” (Knight & Shea, p. 93). Nursing informatics has evolved over the last half decade from a system with only a few abilities to a worldwide technological system used in many hospital settings and physician offices in order to unify healthcare, eliminate error, and allow clinicians to spend less time charting and more time fulfilling job duties.…

A standardized nursing language for documentation of nursing care is important to the nursing profession and to the care providing nurse (Rutherford, 2008). Nursing has always faced by two challenges including, to distinguish nursing contributions in patient care from medicine and to incorporate nursing care into the healthcare to improve the patient’s care. As the development of Electronic Health Records (EHR) and the establishment of Nationwide Health Information, these challenges become more significant. It became essential to identify nursing contributions in patient care and to incorporate elements of nursing care in to the EHR. This quandary was resolved by using standardized nursing terminologies to reflect the uniqueness of nursing…

In 1966, the American Medical Association first founded Current Procedural Terminology (CPT) codes. In the United States, CPT codes are five-digit numbers used as an unvarying style of language used to report medical services in terms of reimbursement (American Academy of Professional Coders [AAPC], 2016). Acute care nurse practitioners (ACNP) need to be proficient in using CPT codes because those codes are directly related to an ACNP’s pay (Richmond, Thompson, & Sillivan-Marx, 2000). A crucial aspect of using CPT…

In 1893, physicians realized the importance and the value of the standardized language (Rutherford, 2008). This recognition increase when the physician realizes how the standardization improved the communication between health care professionals (Rutherford, 2008). With the beginning of the standardization system, medical diagnosis developed many classification and codification diseases to have evidence in the medical practice (Rutherford, 2008). An example of this classification system is DMS-IV for mental disorders diagnosis (Rutherford, 2008). This standardization helps the multidisciplinary team understand the patient’s diagnosis (Rutherford, 2008). In 1973, The American Nurse Association introduce the standardized language and for the years to come has been developed different nursing communication systems as Nursing Interventions (NIC), Nursing Outcomes (NOC). The use of standardized nursing languages enhance communication care not only nationally, but internationally (Rutherford, 2008). This is very important because the help and the care will be the same at any place in the world. This help every nurse to know what an effective care…

Implementation of an EHR and demonstration of meaningful use should not make a nurse's job more difficult. According to Mihalko (2012), “a well-designed EHR should support the nurse by streamlining processes and workflows, assisting with intelligent decision support at the point of care, standardizing documentation, and making data entry and data retrieval faster and more accurate” (p.2). The focus should include medication administration safety, transparency of the nursing process, diagnosis prioritization, and electronic workflow development should standardize and improve communication. By using their voice nurses must begin to expect solutions that improve electronic documentation, communication, patient safety and care (Lavin, Harper, & Barr, 2015).…
