I only had information that was on the patient medication record. The patient was getting their medication in a dossette box (medicines compliance aid). I only had information about his medication and how long he had been on the medication. I did not have any information about his lifestyle and if he is experiencing any side effects from his current medication. Looking at his current medication, there were a few interventions that I noted but I did not know why his medication was changed. I had to make a lot of assumptions as I was not able to gather all the information required. It also made it difficult for me to assess any cautions or risk associated with care of this patient. For Patient two, this is even more important as all the risks should be minimized. There are risks to the patient as the patient is diabetic and from the medication he is on, indicates that he is taking the maximum dose, which shows that his condition may not be well controlled, but because I was unable to speak to the patient I am not able to judge the clinical condition and if his current medication regime is appropriate for him or if any changes are required. Hence, the pharmaceutical care plan may not be tailored to the needs of the patient. Also, due to his dossette boxes being delivered to his home there is an increased risk that medication could be lost or delivered to the wrong address (General Pharmaceutical Council, 2015, 2015). There are many risks associated with providing services at a distant therefore, it becomes even more important keep records of all the activities that take place when providing these services. No records are kept for patient 1 but record of when dossette boxes are prepared, checked and collected by is recorded.
