Upon arriving at the care home, I shut the door behind me, clocked in using my time card and signed the staff log book which is a requirement of the fire safety policy. Prior to starting my shift, I attended the hand over held in the staff office with closed doors to maintain confidentiality and privacy of the residents. The hand over gives me important changes in the resident's health and social care needs, requirements and procedures that need implementing during the shift.…

Effective hand washing is the first step in preventing cross infection and the maintaining of hand hygiene is paramount to its control (Gould et al 2008). However, hands are the preeminent route by which HCAI’s will occur (Elliot 1992). In the perioperative environment there are two types of hand washing techniques.…

After analysing the scenario, I have identified six goals, three of which are immediate and the other three are time critical but not as crucial as the most immediate. The first priority is that Janet (the service user), was found in a bed soaked with urine, the reason for this is that her catheter came out during the night, and her husband (Bob) wouldnt use the hoist to change her sheets and wash her. I feel this to be of immediate priority, as Janet will not only be uncomfortable, but it will have a negative impact on her skin integrity. This adds to the risk factors already contributed to Janets skin integrity being compromised. Other risk factors for poor skin integrity include inadequate oral nutrition, dehydration, lack of sensory perception, decreased mobility or medical conditions that affect circulation. (Hart 2013)…

I am currently studying HNC Health Care and as part of my course I have to complete a graded unit, this will entail three stages; Planning, development and evaluation. This will be carried out while on placement within a hospital setting, within the planning stage I have to choose a patient and assist them with a nursing activity. I have chosen to follow Roper Logan and Tierney twelve activities of daily living the reason for this is that I find it to be the nursing model that is most effective as each activity has its own importance to the survival of life; also it follows the objectives of my graded unit. I have chosen the AL of eating and drinking as the patient I have chosen has severe Rheumatoid arthritis and has lost the use of her hands so she is unable to feed herself so my aim is to assist her whit this AL ”For a variety of reasons a patient may require assistance with the actual activity of eating and drinking, and this should be given graciously and with dexterity so that he will be protected from the needless embarrassment resulting from the lack of independence in this AL”(Roper Logan Tierney.p141). While I am assisting with this task I must remember to respect Mrs A’s dignity at all times.…

For example, Jane was new to the care home, she appeared a very happy and loving lady, a previous stroke had left the muscles around her jaw very weak and she was unable to speak in a conventional way. At every mealtime, Jane would stand in front of the dining room and shout and tell off everyone in the room, she declined to eat and in the end would get so frustrated that nobody was doing as they were told that her aggression would become physical and she would abuse fellow customers and staff. The carers then looked to her care plan, started to have little meetings with Jane several times throughout the days, they would use body language, facial expressions, picture cards, pen and paper and a computer to find out what best suited her to communicate through and also to find out how she was feeling at that particular time of the day. The staff found that Jane liked to write and paint how she was feeling and what she would like to say. From family and friends, they found out that Jane had been a headmistress at a very strict school, she was the person who the naughty children were brought to and disciplined by. One day Jane saw everyone in the dining room and pointed to picture of a class room, then the carers realised she saw the room full of people as pupils. So the carers put together folders and a table that resembled a desk and asked Jane if she would like to…

The two guidelines for behavioral observation that I will describe are conducting preliminary assessment and deciding how many behaviors to record. When the practitioner conducts a preliminary assessment, he or she will review the client concerns and gather information to begin helping the client work in that area(s) of interest. When the practitioner decides how many behaviors to record, their goal is to keep the process simple. One should not expect the individual recording the information to record more than two behaviors.…
