Although the strength of the research could predict that, “participants’ behavior was viewed during critical events, the lack of consistent observation leads to the assumption that nurses’ skills are not reliable in caring about patient safety” – can be true but limitation also too be addressed. One of the limitations of the study is the need to simulate the real world behavior and reflect them in the actual care unit. Even then there might be variables missing compared to the real world. Another limitation is caused due to the intentional creation forced errors. They might not be reflection of normal condition (Miller et, al. 2009).

With two behavioral markers of detailed information like the verbalization of changes on maternal and fetal condition were only observed 65% of theme, the more requested back up only 62% of the time even when it was needed. More importantly, verification of critical information had happened only 19% of the time. However, nurses’ observed very well in introducing themselves and prioritizing tasks 90% of the time. Closed-loop communication is an essential feature of high reliability and one of the key 2009 patient safety goals of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO 2002). The researchers approached the grounded theory, “An open, exploratory approach was used to identify recurrent meaningful concepts through systematic, inductive analysis of content” (Polit & Beck 2017, p.17). The article has the framework of conceptual underpinning by a shared mental model enables the team members to reach common understanding through communication individually to conclude patient condition and the care
