Readiness Assessment Case Study
WHAT MAKES READINESS ASSESSMENT TO BE SEEN AS A PROBLEM AT VIKURUTI REHABILITATION VILLAGE.
During my community mental health rotation at the Vikuruti rehabilitation centre I noted that, there is a problem in the readiness assessment before patients transferred/ referred from MNH to Vikuruti rehabilitation centre. First I heard one of the staff who mentioned readiness assessment as a challenge when we were asked them about the challenges which they met as a rehabilitation. She said that, “I don’t think if MNH staff did a proper assessment of those patients who brought here to see if they are really ready to come for rehabilitation, because some patients when they reach here they complain and said they are not ready to stay here and if they

He said that, “they didn’t ask me anything, is my mother, my sister and my brother who tell the doctor and other workers at Muhimbili Hospital to bring me here because they don’t want to stay with me in their home because I used to drink and smoke cigarette”. I tell him he has been taken there not for punishment, but for him to regain the skills which it has been lost due alcoholism and the cannabis which he was used to smoke before started medication and decide to stop using it. I told him that rehabilitation program will help him to regain and go back to his work of teaching, but he insisted that he wants to go home he don’t like to live there. Then I tell one nurse that, the patient is insisting to go home, they have to take care because he may abscond, and the nurse said it is true he don’t want to stay there since he has taken there. The following two days when I went there I found he has absconded from the rehabilitation, the nurses called the relatives at home and they said he went home.
Then I realize it is true that readiness assessment is a challenge for some patients, not all because others (two patients) they said they have been involved in the discussion and asked if they were ready to go for rehabilitation or

Block manger said readiness it is usually done by the all team members of the psychiatric unit (Psychiatrist, mental health nurses, psychologist, occupational therapist, social worker and community mental health nurse), patients and patient’s relatives before patients transferred or referred to the rehabilitation centre. She said the nurse from the Vikuruti Rehabilitation village is supposed to sign the assessment form before she/he take the patient to the rehabilitation village, so she is wondering how can Vikuruti staffs report the readiness assessment as a problem while they are the one who take the patient to Rehabilitation, why don’t they refuse to take the patient if they see the patient is not ready? Then I tell her even some of the patient’s reports that, they were not ready to go there and their relatives and health care providers are the one who decide for them to be taken there. She said for the patients some time they may say that after see the environment was not good for them to stay

When an organization is trying to reorganize and create new programs, they will use an evaluation plan to do this, an evaluation is a very important step in the development and implementation of the organizations program and projects they are trying to develop. It’s important to have a solid foundation to carry out the mission, goals, and the objectives, and it’s also important to have a way to define success and failures through the proper use of evaluation techniques, appropriate reporting, and having the right tools to carry out the programs. It can be challenging for an agency to decide if they should use a process evaluation or an outcome evaluation plan. The main objective of an evaluation technique is to provide for the state the agency is in, tell how – well or how poorly the programs are working, what may need to be done or changed, and how they are keeping up to meet their goals mission, and objectives.…

The History of the United States of America would have differ greatly if not for the founding of the Pennsylvania Colony. One of our most sacred beleifs might not be the same if not for the Quakers in Pennsylvania. While most of the other colonists did believe in the idea of freedom of religion they were only okay with it if you were practicing their religion. Another major change in American history the Pennsylvania colony had was the advancement in technology and Higher education. While I believe that Americans would eventually get on the same path as the Pennsylvania colonists it would have taken much longer which could alter history greatly. Without the Colony of Pennsylvania the Scrolls of American history would have been completely…

Assessment is described as ‘the first step of the nursing process,’ Wolters et al. (2010 p.36). She pointed out that it is the act of collecting, organizing, evaluating and documenting information about the client’s wellbeing, while Varcarolis (2011) mentioned that an assessment is an interview which examines the mental state of a patient. Furthermore, the National Institute for Clinical Excellence (NICE 2009) guidelines give emphasis to early access to assessment and treatment for people with psychological problems.…
