With more than 3 million members, the nursing profession is the largest segment of the nation’s health care workforce. Working on the front lines of patient care, nurses can play a vital role in helping realize the objectives set forth in the 2010 Affordable Care Act, legislation that represents the broadest health care overhaul since the 1965 creation of the Medicare and Medicaid programs. A number of barriers prevent nurses from being able to respond effectively to rapidly changing health care settings and an evolving health care system. Nurses are in an ideal position to propagate changing within the health care workforce, with three millions nurses to effectuate change, and the objectives within the Affordable Care Act (ACA) providing the largest overhaul within medical care since the inception of Centers for Medicare and Medicaid (CMS) in 1965 (Institute of Medicine [IOM], 2010). In 2011, the Institute of Medicine (IOM) partnered with the Robert Wood Johnson Foundation (RWJF) to publish The Future of Nursing: Leading Change, Advancing Health. This report is a collaborative report written by a committee of scholars nationwide. Its purpose is to put forth recommendations that allow nurses to “(1) ensure that nurses can practice to the full extent of their education and training (2) improve nursing education, (3) provide opportunities for nurses to assume leadership positions and to serve as full partners in health care redesign and improvement efforts, and (4) improve data collection for workforce planning and policy making” (The Robert Wood Johnson Foundation and the Institutes of Medicine [RWJF and IOM]. She accuses the company of engaging in discriminatory policies in which a man of Anglo descent was chosen for the position instead of her. In my paper I will talk about my research into the Hispanic culture, the role of a women in the Hispanic culture as well as women in the workplace. The atmosphere that the nurses work in are at times described as one of inappropriate skill-mix, low nurse to patient ratios; a lack of involvement in decision making; managing constant changes; issues with shift work; leave and pay; and increased patient expectations. Such problems add upon the workloads and stress levels of nurses leaving them feeling undervalued with a loss of interest to continue. Recent findings show a 1 to 1.4 % per month nurse attrition rate in just one state (New South Wales). High rates of nursing attrition where staff willingly leave or transfer between positions in nursing, or leave completely for another profession is an important priority for the health system for a range of reasons. Firstly, attrition is costly, estimating at $16,634 per nurse in Australia. The United States is one of the most ethnically and culturally diverse countries in the world. Many people of different customs and religions share the same space. Yet, the healthcare workforce does not represent the nation’s diverse population. Physician Assistant (PA), specifically, still remains predominantly white.
