Root Cause Analysis: Import Raw Materials Company
Before 3 years, I worked as an admin in Import Raw Materials Corporation the team's lead gave me his office, firstly I thought that he love me. It took two months to understood that I was wrong, he was giving another manager the wrong intelligence of me, but at an Outside work, the manager told me what was happening. When this problem got to my manager, I was already conscious of everything, I was able to solve the problem and crush any hearsay or wrong intelligence about me when I went to see the boss of the company. I was annoyed and my response to challenge him in front of my Director and another manager, but I decided to give myself a some of the time to think about how I dealing with this problem. Then I asked him several questions about administrative work, his attitude changed completely. Then he started asking me questions and he noted my points which I focused them on working and solving problems from this week I've selected the Root Cause Analysis.
Here is my analysis of the above situation:
Physical Causes: The team lead was a very loyal employee who had been with the company since its beginnings and time and time and again, he was overlooked for a promotion as a supervisor.

Human Causes: A few newly promoted managers thought it was best to hire someone from the outside with fresh and innovative ideas, thus overlooking at someone who was eager to learn and move up in the company.
Organizational Causes: This type of thinking by new management was flawed because the message this relayed to everyone else conflicted with the mission, vision and values that the company claimed to observe and uphold.

I saw a good employee that was very frustrated and was taking it out on me.

Lack of trust, frustration, talking untruthful things behind the bosses (me) back, sabotage

Step 3: Identify Possible Causal Factors
What sequence of events leads to the problem?
New management did not think he could do the job even though he had been doing it for 7 years They ended up hiring me, thus putting salt on the wound and overlooking at him for management
Step 4: Identify the Root Causes
Why does the casual factor exist?
Because an employee feels unappreciated after 7 years of loyal and arduous work.
What is the real reason the problem occurred?
New management that was not aligned with the company's mission, vision and values.
Step 5: Recommend and Implement Solutions
What can you do to prevent the problem from happening again?
I ended up talking to the President of the company to explain the entire situation. This did not earn me any popularity votes with many, but it needed to be done as the President was not very happy with what was
