Sample MRI Case Report
DOI: 5/29/2015. Patient is a year old male truck driver who sustained injury while he was driving when he felt a pop in his back and felt extreme pain. Per OMNI, he was initially diagnosed with lower back strain.
Based on the latest medical report dated 03/28/16 by Dr. Sanz, the patient complains of unchanged moderate to severe pain in his lumbar spine, described as achy, dull and stiffness. He reports worsening left-sided lumbar spine pain with radicular symptoms in the bilateral lower extremities, left greater than the right side.
