DOI: 7/28/2000. The patient is a 46 -year-old female professor who sustained a work-related injury after being rear-ended while waiting for a pedestrian to cross the street against a green light. As per OMNI, the patient is diagnosed with chronic low back strain and chronic occipitocervical dysfunction.
Ortho AME Dr. Sommer opined that IW is at MMI on 12/03/01. Future medical care includes office visits and medications. Neuro AME Dr. Gupta has opined that the IW is P& S on 07/01/02 with future medical for treatment of headaches. Treatments include Botox, medial branch blocks, massage, pool therapy and medications.
Urine toxicology screen obtained on 09/04/15 revealed positive for opiates, hydrocodone and hydromorphone.

Motion is painful. There is pain over the facet joints, worsened with loading maneuvers. Active range of motion shows extension and bilateral lateral flexion to 10 degrees, bilateral rotation to 30 degrees and flexion to 55 degrees, with pain.
Assessment includes long-term (current) use of opiate analgesic, myalgia, cervicalgia, chronic pain due to trauma and headache.
Treatment plan includes medication management, acetaminophen, complete blood count (CBC) including differential count and platelet, Chem 20 panel, Gamma-Glutamyl Transferase (GGT), hydrocodone and metabolite serum, urine drug screen (UDS) and alcohol and complete urinalysis.
IW will be periodically monitored for adherence with urine drug screen, CURES and routine laboratories. She will follow-up in two months.
Per the request for authorization form dated 01/22/16, all patients with prescribed medications will have routine CBC, Chem Panel, thyroid stimulating hormone, UA and UDS twice a year. Male patients will also be screened for prostate-specific antigen (PSA) and testosterone levels. Patients will also receive random UDS
