The overall goal of the grant proposal is to get funding support for a patient accessible portal which allows diabetic patients to become proactive in their health and wellness to enhance their quality of life. The portal will have various features such as interactions with physicians, ability to record health vitals, monitor health status, follow recommended lifestyle changes to improve quality of life, and alert patients before serious conditions may arise. It is anticipated that the portal will encourage the development of healthier lifestyle attitudes. Diabetes can be an expensive and deadly disease if not monitored correctly, however it is easily manageable with the right resources. Therefore, this proposal is about innovation and technological advancements to help reduce or eliminate diabetic risks. The research duration is one year with an expected sample size of 40 will be collected and samples will be taken at day 0, six months, and 1 year. After researching diabetes, the findings were that African Americans are the ethnic group that is most affected by the disease. 4.9 million African Americans have been diagnosed with diabetes and the numbers continuously rise every year. (Centers for Disease Control, pg.3, 2011) Cardiovascular disease, high blood pressure, and strokes are a major cause of death linked to this disease at an alarming rate of 67%. Psychology in diabetes care is important because it is linked as well with the illness. The psychologist provides direct services to the patient by promoting health behaviors and treatment of psychological problems and provides counseling to patients that do not adhere to the diabetes regimen. They help them to understand the severity of diabetes and its symptoms and outcomes as well as provide knowledge. Helping the patient with depression, anxiety, eating disorders and additional burdens from everyday life effects health as well and the team of professionals work with the patient because these factors can cause elevation in blood sugars and interrupt routines such as eating and sleeping. The aim of this essay is to consider approaches to promoting the self-management of a long-term condition. The focus will be linked to module content and based on a patient that I have provided nursing care for during my clinical practice. There will be a brief introduction of the patient and the context in which I was involved in their nursing care. The pathophysiology of the chosen LTC along with the related physical, psychological and social needs of the patient will be considered. There will be a discussion of principles of self-management that will link to national LTC policies. Nurse educators are charged with teaching student nurses how to become self-aware and to use this knowledge therapeutically and in leadership activities. Self-awareness has been widely accepted as an important part of contemporary nurses' repertoire of skills and has been said to be an important factor in a successful nurse-patient relationship. It has been identified as a factor in empathy, supporting the notion that self-awareness leads to a gentler way of being and compassion results. However, there is a lack of empirical evidence related to how students actually develop self-awareness within the context of nursing education. This qualitative study focused on describing the elements of who, what, where, when, and how which influence the development of self-awareness in nursing students. Purposive sampling was used and data collection through two in-depth, face-to-face, audiotaped interviews continued until saturation occurred. Guided by naturalistic inquiry and interview research, ten baccalaureate nursing students described how they developed self-awareness as part of their nursing education program and identified the contributing contextual features. Through content analysis categories of external and internal influences on self-awareness development within the context of nursing education emerged. The process of self-awareness development was identified as occurring within the context of both interpersonal and intrapersonal interactions. External factors which influence self-awareness development include socially and emotionally significant relationships, facilitative actions of others, transformative events, and pedagogy. Internal factors that emerged include religion/spirituality, and personality/developmental issues. These findings have heuristic value for enhancing self-awareness development within the context of nursing education.
