Recording information –

As a support practitioner all writing and recording must be written accurately, factually and must be relevant. All information and entries are to be dated and signed . All information and paperwork must be legible and there is to be no use of abbreviations or jargon. Storing information-

It is the responsibility of every employee to ensure all confidential records are kept secure. These must be stored in a locked cabinet or locked area which is accessible for each person we support to access. Reporting information

Reporting information can be done using the Staff Communication book which is open to all staff and is not confidential. Appointments and daily activities are communicated using the Daily Dairy and Staff Handover Sheets, and each day the Daily Notes are written to help maintain communication between staff and a continuous delivery of service.

Communicating/Sharing information-

Privacy requires the facts or information shared by someone with another party to remain private. Every individual has the write right to privacy, and for their information to be kept secure and not disclosed without the permission of the person who gave them. The Social work Inspection Agency released a publication in January 2010 named: Effective Management of Social Work Recording, in which it states that "This can require some professional sensitivity when workers write down what they consider the necessary information about a person they should be aware of how it would feel for that person to read what they have written."
