In the first phase I will address cost-cutting. Our goal is to cut-costs in the first quarter by $750,000. The cost cutting options I have chosen for EHC are, reducing agency staff and changing the skill mix within the organization. These two cost-cutting strategies had the least effect on patient care and saved the organization more money over a longer period of time. Having agency personnel is twice the price of having hospital staff. Also, Hospital nurses have more knowledge of the patients and give better patient care because of the familiarity. Recently the state Department of Health and the federal authorities for Medicare/Medicaid visited our facility. They have assessed that our residents are in danger of inadequate nursing staff-to resident ratios and lack of a qualified nutritional dietician. The facility has been given 90 days to increase staffing by eleven licensed nurse aides, three registered nurses and a registered clinical dietician. The survey team will return in 80 days, unannounced and if these deficiencies are not resolved, they will have no choice but to close the facility. The other challenge is competing with two hospitals in the community that are continually seeking the same types of health professionals. We should consider the fact that they have greater resources to attract and retain staff. Since we have only 80 day to recruit new staffs we need to come up with a short term plan that attract nurses to our facility and also a long term plan to retain staff. Our facility has been part of the community for nearly fifty years and has the advantage of an excellent reputation and community support. In 2005, the Bureau of Justice Statistics did a study that of the almost 3.5 million crimes in America violent crimes committed against family members , 49% of these were crimes against spouses. A police officer how responds to a domestic violence call can be a life or death situation. They must be able to recognize when an abusive situation is happening even without physical evidence. In other cases the abuser will not answer the door if he see the officer at the door. In the 1980’s the response was very little to the domestic violence. Then manner in which new graduates are socialized and oriented in the unit or facility is essential. Our unit adopted this program because we found/heard that many new graduates coming out of nursing school felt like sometimes the more seasoned nurses were sometimes unfriendly or "like to eat their young". The act of assisting suicide is assisting or encountering the other person to kill themselves. There are several classifications of euthanization; active and passive. “Three states have passed laws legalizing assisted suicide in certain limited circumstances. Hall theory contains three independent but interconnected circles, the core, the care and the cure. Hospital administrators need to work collaboratively with recruiters, nursing directors, and managers, to improve and support the work environment for nurses. The hospital where I have dedicated my twelve year nursing career has gone through a massive change. When I started working there in 1999, it was a small, independent 168 bed community not-for-profit hospital. The expectations of healthcare today were and continue to be a costly venture to a non-corporate facility, which is why the hospital was struggling financially. It simply could not meet the needs of the growing community. As a result, it was purchased by a corporate healthcare entity based in North Carolina that owned several other hospitals. This merger brought about massive changes to the community hospital. Their policies, practices, benefits and overall mission and vision have been redefined. The introduction of the newly adopted mission, “We exist to improve the health of communities, one person at a time” (Novant health, n.d.) and, the vision, “We, the employees and our physician partners, will deliver the most remarkable patient experience, in every dimension, every time” (Novant health, n.d.) were one of the first initiatives undertaken.
