During the orientation phase of my interaction with the client, I requested the client to write down three (3) things that he could give to himself as gifts (realistic and unrealistic). The client came up with three dreams/goals that he wants for himself; to have two children, to marry his girlfriend, and get a stable job to support his family. contentment than can be part of his treatment plan. This may help him to be able to cope with his triggers on depression.
Other intervention that may work is the establishment of an environment that promotes free expression of feelings and concerns. This will aid in the development of trusting-relationship of the client which is important during the orientation phase. If I could go back and do things differently I will dig deeper on the feelings that my client projected during the orientation phase, rather than thinking that it is too early for such burst of feeling. I should have encouraged the patient to express freely his feelings and concerns in order to be a more effective advocate, even if his burst of feeling will make me uncomfortable.
b. Comment on any change or movement seen in your relationship with your client and the possible reasons for this; Before the therapeutic relationship was fully established, the client was transferred to the Highlands facility at 0900 on December 09, 2015 as per request by the client and his family in order to be closer to his family and girlfriend residential area.
c. What do you think would be important for this client's progress to continue? If you were to continue working with this client for the next several months, what would your plan of action be?
If I continue to provide service to this client for the next several months, my plan of care will focus on suicide ideation prevention. The interventions will mainly emphasis on suicide assessment and provide client and/or family education on the importance of taking medications on the regular basis even if the symptoms of his diagnoses subsides. Another intervention is to support client’s defense mechanism and coping skills that works for him may lead to a positive outcome in dealing with his disorders.
The importance of the identification of meaning and purpose in life that reinforce hope, peace, and contentment upon discharge may be beneficial in strengthening the client’s coping skills.
This may lead to the prevention for another suicide attempt in the future.
d. What ethical dilemmas (if any) were encountered by yourself and/or the treatment team in providing care to this client, and how were they resolved? If you were assigned this client again, knowing what you know now, in what ways would you change your approach?
I did not encounter any ethical dilemma when I provided care to this client; however, if I were assigned to this client again I will focus on the establishment of therapeutic client-nurse relationship in order to be able to support client’s strength that will contribute to the development of his self-worth, and the strengthening of his spirituality. His numerous suicide attempts are evidences that he’s spiritually distress.
e. What cultural issues and personal biases emerged during your work with this client and how did you resolve?
I did not have any cultural and personal biases that emerged during my service with this client. As a Christian nurse, I believe that God expected me to serve my clients in a nonjudgmental
manner.
