Toxic Claim: Case Study
I’m not sure what to do with this appeal. In my SOW, I clearly stated that the claims should be reprocessed F3 and F4 with timely filling bypassed. The rework team is refusing to run the claim because out of 2795 claims, she found 19 (at the time she told me 1) provider (s) with the wrong provider ID#. From my understanding, that is why they have the “skipped” claims section in macro. I could take the 19+ from the 2795 and create another project. Meanwhile, the appeal is ageing causing me to keep having to send it back for the one offs. The other alternative be to test all 2795 claim which be outrageous. This defeats the purpose of having a SOW, if they don’t follow the instructions provided to them. I did my part and tested some of the claims

If a patient visits a specialist but did not receive the required authorization prior to the visit, the claim may be denied, resulting in the provider’s need to appeal the claim (Jacob, 2001). Healthcare employees who handle billing and claims must be certain that all of the information they have for each patient is correct and up to date, and that they receive all necessary authorizations prior to performing any procedures. Additionally, insurance clerks have to be certain that they are using the proper procedure codes and not unintentionally over coding. By filing an appeal, the claim can be paid when it was previously denied, reduced, or down coded. After the appeals process and decision, if a provider or patient is still not satisfied, the appeal can be taken to an outside authority, like a state insurance commission…

Further, the Union will have to prove that the company violated the contract by outsourcing the work of janitors prior to negotiating at the bargaining table to retain the janitorial jobs. The company is…
