Unit 9: Values and Planning in Social Care
Task 3

You must produce fact sheets relating to two of the individuals in the case studies which consider how you can plan to support them. Within these fact sheets you must:
Identify the processes and assessment tools involved in planning support for individuals with different needs who use social services (P2)
Describe how three key professionals could be involved in planning support for individuals (M2) within the case studies
Assess potential issues which could arise from the involvement of several professionals in the planning of support for individuals (D2)

Throughout this task, reference to the two case studies must be made

Identify the processes and assessment tools involved in planning support for individuals with different needs who use social services (P2)

To achieve this pass you must identify the tools and processes used to assess individuals before planning support.
You must include:
A description of the cycle of assessment and planning, and describe each of the following stages:
1. Referral
2. Assessment of holistic needs and preferences
3. Identification of current provision
4. Implementation
5. Monitoring and reviewing
6. Involvement of key professionals
For each stage give an example from the case studies.
Describe what assessment tools are and how they are useful in planning support for individuals
Choose two assessment tools for each case study from the list below. Describe what they are and how they would be useful in assessing the individual
Checklists
Diaries of professionals
Records of incidents and accidents
Use of appropriate questions
Diaries of individuals who use services
Observations
Personal histories
Identify two approaches of how support plans can be implemented

Describe how three key professionals could be involved in planning support for individuals (M2) within the case studies

To achieve the merit you need to identify three key professionals who could be involved in either of the case studies (both case studies need to be included) and then:
Describe the job role of the professional, and include references where appropriate.
Describe what the professional would do for the individual
Describe how the professional would help in planning support for the individual
Finally, include a concluding paragraph on how professionals work together across professional boundaries for planning support for each of the individuals (think about why this is important)

Assess potential issues which could arise from the involvement of several professionals in the planning of support for individuals (D2)

To achieve the distinction you need to consider both the positive and negative aspects of potential issues that could arise from the involvement of several professionals in planning support for the two individuals from the case studies.

You must include the following, and make reference to both case studies throughout:

What are the positives of several professionals working together? (E.g. use of expertise and team working)
What are the negatives of several professionals working together? (E.g. Professional pride, problems with communication, confusion for service user)
You must assess each of the issues- stating why it is an issue (positive or negative) and what effect it has on the individual?
How could the issue be used as a positive example, or how could it be overcome?
How would each of these issues have an effect on multi-agency/ interagency planning for the individuals?
REMEMBER as it is a distinction each point needs to be in depth, and references need to be added where possible
