After the client provides their registration information, I would start with questions in order: the client family history, socio-economic history, developmental history, emotional/psychiatric, medical history, substance use history, and then the symptom checklist. On page two, I would condense the section containing the symptoms to eliminate the repetitiveness and provide simple terms. Because this intake form is organized in clusters, it does not provide much space for documenting information. I would space out the sections providing more space for the clients to record their information. This might create more pages but all the information would be able to be addressed.
