The assessment wheel is a tool used to guide and organize data researched on Black Hills. The assessment wheel allows for health promotion and reduction in disease and death within the community researched. This core set of interventions will examines all factors that promote health and prevent disease.
Physical Environment When observing Black Hills the boundaries within the neighborhood were apparent. The bordering roads consist of Godfrey Ave, Hall Ave, Freeman Ave and Market Ave. The area is approximately is approximately 0.40 square miles with the main streets Dorchester Ave, Kensington Ave, Woolsey Dr., and Oxford St. Some of the natural borders are the Grand River and the Hillside. Some of the institutions in the surrounding area are Cesar E Chavez Elementary School, City Middle/ High school, John Ball Park School, and Grand Rapids Community College is approximately a 10 minute drive. The percentage of children enrolled in K through 12 schools is 39.2 %. According to statistical analysis people older than 18 the education levels are below national standards 42.8 percent of people completed the 8th grade. 43% of residents graduated high school. Of the amount of residents that have attended college 3.2% obtained an associate’s degree and 1.7% have received bachelor degrees. There are two community centers in the area. The Black Hills Community Center and The Hispanic Community Center both of these centers offer recreation for children and adults within the area. Some other bonuses of having these community centers are for the classes that are offered. Each year the Black Hills community center offers classes for adults to receive their GED’s. The community is faith based United in Christ ministries plays a large role in this community and the two churches within the community help those who are poverty stricken with food and often help by offering some needy families coats and other clothing
