Assignment 2: A Case Study Of Mr. Short's Self-A
PM and CM met with Mr. Short regarding his medications/refills and to check on his well-being after an (incident report) stating that Mr. Short was exhibiting abnormal behaviors; such as, slurred speech, self-confessed hallucinations, and not able to keep his physical balance while walking. Mr. Short’s medications were reviewed and updated in the system. Mr. Short was given a pill organizer and instructed to go to HSC/MHMR to have a med check conducted. Additionally, CM will be following up with Mr. Short’s MHMR CM – Missy, via e-mail/phone call.
