Ayden Case Study
Circumstance: Ayden will maintain contact with medical team monthly. MS. Smalls (MHP) and Mrs. Wigfall (MHS) review and sign the current Individual Plan of Care (IPOC)
Action: MHP and MHS review the goals, interventions and overall progress for the past 90 days. MHS and MHP sign the updated IPOC. MHP confirmed the next appointment. MHS report no illness in the past week. Ayden is stable and doing well.

Plan: MHP will provide support to MHS and continue to ensure Ayden well-being and safety
