Hrm/531 Work Plan
The purpose of this assignment is to outline a plan of care for three residents, who, for the purpose of confidentiality within this report, will be referred to as L, J, and F, from the start of my shift at 0645 hours until lunch is served at 1200 hours. The time line will also include all work breaks taken, so as to be as accurate as possible. The work plan will include descriptions of how the comfort and dignity of each resident was upheld while care was provided as well as the safety practices associated with each activity. In addition, before the order of care for L, J, and F is described, I will include a brief overview of each client's significant individual care requirements.

0645 – Arrive at the facility. Change out of my street shoes into my work shoes. Meet with the instructor and peers in the parking lot. Hand in the previous day's journal to the instructor and have a quick debrief with the group.

0700 – Enter cottage #6. Wash hands. Join the HCAs for shift report. Enquire as to whether there were any changes in my residents statuses since my last shift. Once I have received any updates, I will begin to prepare for care.

Perform a check for safety and comfort. Turn the resident so the her legs are over the edge of the bed. Lower the lift so that the Resident is just sitting on the bed and so the there is still tension on the straps of the sling. Put on the resident's shoes, being careful to avoid touching the bottoms of the shoes. Raise the lift and position the client over the w/c. As you lower the client into the w/c, push on the crossed straps, hold the bar away from the resident's face, and guide her feet, meanwhile, your coworker tips the w/c to ensure the client lands is positioned properly in the chair. Ask L if she is comfortable in that seated position. If she is, unhook the sling from the lift, and tuck/tie the sling away, as it will remain under her until she lies down
