Case Study: Acceptatory Phase III Trials
Design considerations are critical for confirmatory Phase III trials because they form a large part of the basis for regulatory approval and potential changes to clinical practice guidelines. For this reason, confirmatory trials should be adequate and well controlled, typically with placebo or a standard therapy as a control and a clinical benefit endpoint, or a validated surrogate as primary endpoint for full approval. For accelerated or conditional approval “likely surrogate” endpoints may be used.

She became lethargic within one hour. She was immediately treated with “10mg activated charcoal, 3 h post ingestion”. The patient then went unconscious but did not need any tubes or extra ventilation. On arrival, her pulse was 110, blood pressure was 106/60, RR (melatonin level) was 20, body temperature 98.3, pOx 98 (RA). She had a disconjugated gaze. Her skin appeared dry, pink, and warm. Bowel sounds were decreasing. An ECG with 12-lead showed a sinus curve of 110. She was in stable condition with consistent vital signs and slowly awakened throughout a 24-hr period. On the second day, she slept for excessive periods at a time. She also suffered “truncal ataxia” while in the sitting position, which still existed during walking and standing up the next day. On the fourth day, she showed trouble with fine motor skills, but was discharged. The girl was monitored for seven days as symptoms receded and no abnormal condition resulted…

The case involves Dr. Barret Rollins, Chief Scientific Officer (CSO) for the Dana- Faber Cancer Institute, and Diana Cohn, a member of the institute board of trustees. On October 14, 2010 Dr. Rollins received a call from Diana Cohn expressing her concern about the lack of progress regarding the NIH Grant, largely due to miscommunication and lack of collaboration between centers of the institute. Diana Cohn’s remarks stem from dealing with her husband’s battle with Glioblastoma Multiforme, the deadliest type form of brain cancer. There was no cure for her husband’s cancer but research was conducted by the Institute to find a potential cure or treatments for individuals who suffered from the same disease. After hearing Dana Cohn’s…

She is not a qualified candidate for Phase I Trial because phase I trial is meant only for healthy individuals and the researchers test a new drug or treatment in a small group of people for the first time to determined its safety and safe dosage and also to identify side effects. Phase I trial will be a waste of time with a person like Mrs. Wilkins who is in her final stage of their life, because nothing good might can come out of it or there will be not enough time for the Doctors to get the result or information that they are looking for. In addition to the above, her immune system is already too weak and with the new drug, it might hasten her death because nobody knows anything about the drugs yet.…

The researchers interviewed 21 adolescents between the ages of 13 and 19. The study took place in Northern Ireland, where they found participants from three diabetes clinics within the area. The participants had to be diagnosed with diabetes for at least one year, and spoke English. The focus groups were recorded using a digital Dictaphone; the recordings were transcribed word for word and analyzed through thematic content analysis. The study presented some limitations including smaller sample size, and lack of demographic data which creates the possibility for selection bias. The results showed that many adolescents found difficulties with social acceptance and knowledge of their condition to attain optimal control. Many participants expressed that complications of diabetes should not be addressed in these programs because they feel it is a method of trying to persuade compliance and unrealistic dietary expectations. Another issue explored was the need for health professionals to acclimate to the lifestyle of communication, such as text messaging, which they felt was a…
